Persons who are in quarantine due to exposure to a person with symptoms of COVID-19 or exposure to a person with a lab-confirmed positive case of COVID-19

Distance Learning Notification

  • Example: John Doe, Jane Doe, etc.
  • Please provide an email and phone number we can best reach you at. (Example:, 952-442-0000)
  • Please provide the days in which your student(s) will need to distance learn. Note: If your family celebrates the holidays normally or will be traveling during school breaks, we respectfully expect you to keep your child(ren) home for a minimum of 14 days after the latest date your family participated in that event or trip.
  • Please reply NA if none apply to your family.
